and send the following documents to Passport Deport

Visa Application Form
Valid Passport (must have at least 6 months of validity left and have at least 2 blank visa pages) and a copy of passport biographic page
1 Passport Photo glued on Section # 1 (personal details), left side of the visa application form
Copy of airline ticket shows Entry and Departure of Korea
Copy of Bank Statement : Balance should be over $3,000
Copy of Hotel Information( or Address iin detail and Contact Phone Number in Korea)
Non US citizens must also submit a copy of : valid US permanent resident card or (a) valid U.S. visa (b) I-94 record (www.cbp.gov/i94) (c) I-20 or DS-2019 that has been recently signed forsigned for travel (F-1/2 and J-1/2 visa)

Passport Depot Fees

Please send COMPANY CHECK, CASHIER’S CHECK, MONEY ORDER made payable to “Passport Depot”. You may also pay this fee by CREDIT CARD by using the Customer Information Form.

Return Shipping Fee

To receive your passport for overnight return delivery, please add $49.00 (or $69.00 for Saturday delivery) to your check/money order or credit card payment to Passport Depot, or you may include your own Airbill with your FedEx Account number.

Original documents proving applicant’s business activities such as: L/C, Invoice Contract, company letter on letterhead stating purpose, length of stay, and responsibilities
Non US citizens must also submit a copy of : valid US permanent resident card or (a) valid U.S. visa (b) I-94 record (www.cbp.gov/i94) (c) I-20 or DS-2019 that has been recently signed for travel (F-1/2 and J-1/2 visa)
